Fecal Microbiota Transplants Show Promise in Treating Chronic Insomnia

A recent study suggests that fecal microbiota transplants (FMT), commonly referred to as "poop pills," could be a potential avenue for adults struggling with chronic insomnia. The small-scale research indicates that after undergoing FMT, participants exhibited a more diverse gut microbiome, leading to notable improvements in sleep patterns.

Conducted across seven hospitals in China, this study involved 80 adults diagnosed with long-term insomnia. Rather than suffer from other sleep-disrupting health issues like migraines or sleep apnea, these participants engaged in a controlled trial to assess the effects of FMT on their sleep quality.

Half of the participants underwent a preliminary antibiotic treatment to deplete their existing gut bacteria before receiving 60 FMT capsules derived from the fecal samples of healthy, well-sleeping donors. Two weeks post-transplant, they took an additional 20 capsules as a booster. In contrast, the control group received placebo pills both in place of the antibiotics and the transplant capsules.

Objective sleep assessments, including polysomnography to monitor brain activity, heart rates, and other indicators, revealed significant differences in sleep efficiency. The FMT group saw their average sleep efficiency rise from 79% to 93% one month after the transplant, while the placebo group noted only slight improvement, moving from 85% to 87%. Participants who received the transplants also experienced fewer nighttime awakenings, with their awake time after initially falling asleep dropping dramatically from 79 minutes to just 17 minutes.

Questionnaires administered after the treatment indicated that improvements in sleep persisted for up to six months, indicating that the impact of FMT may extend beyond immediate post-treatment effects. This durability of improvement has caught the attention of sleep experts, like Dr. David Gozal, who noted that the findings align with ongoing research suggesting the gut microbiome's role in regulating sleep-wake cycles.

Gastrointestinal Health and Sleep Quality

The study demonstrated a favorable safety profile, with only three out of forty participants reporting mild side effects such as nausea and transient fever. However, experts caution against overgeneralizing the findings. Since the trial involved a relatively small and homogenous group, the results may not be widely applicable. Furthermore, sleep quality was evaluated only once after a month, raising questions about the long-term effects of FMT.

Kenji Hashimoto, a forensic mental health researcher, emphasized that because the treatment protocol involved both antibiotics and FMT, it’s challenging to discern the individual contributions of the gut microbes. There’s also room to explore how microbial communities beyond the gut, such as those in the oral and nasal cavities, might affect sleep quality.

Sleeplessness and gut health share a complex relationship, where gut-derived chemicals can influence brain functions related to inflammation and sleep regulation. Notably, changes in gut microbiota due to the FMT led to significant microbial shifts, contrasting with the placebo group that saw no changes. This finding hints at the potential of FMT to reshape the gut environment positively.

Despite these promising outcomes, not all participants experienced the same benefits from FMT, and those with varied response patterns often shared similar gut microbiome profiles beforehand. This variability suggests that initial gut microbiome composition could be a key factor in treatment efficacy, underscoring the need for further investigation. Experts, including Harriët Schellekens, call for more research to delineate the biological mechanisms that underpin these improvements.

While the potential of FMT as a treatment for insomnia is enticing, it remains an experimental procedure. Schellekens advises that until more large-scale and detailed studies are conducted, FMT should be approached cautiously and only within well-controlled clinical trials.

The researchers behind the study did not respond to requests for comments prior to publishing.
